On a foggy evening in June 1998, a United 747 fully loaded with fuel for a 14 hour flight from San Francisco to Sydney, Australia experienced failure of its number 3 engine on takeoff. The plane, carrying 307 passengers and loaded with 400000 pounds of fuel, barely missed San Bruno Mountain clearing the top of it by less than 100 feet!
