What plane is it? Maybe I can try to reproduce. (Freeware? = where can I download?)

What version of RE (v1.1 or v1.2) are you using?

Did you do any changes to the RE modules except changing the parameters (in the individual modules for v1.1 or RE1_Parameters_v12.xml file for v1.2)?

With the MP RPM module indicating complete failure (red in the status panel), the engine should be dead, no matter what the Power limitations module indicates.

Thinking about it: I'd like to check whether one of the other modules is fighting with the MP RPM module.

Try the following:

Move all modules except "RE1_EngineMPRPMLimitations_EngineRoughnessFailure_v12.xml" and "RE1_Parameters_v12.xml" (if using v1.2) out of the "RealEngine" folder to another folder for backup.

Then restart a flight with full MP/RPM and look whether engine 1 fails the same.
