PhrogPhlyer Posted July 4, 2023 Share Posted July 4, 2023 Again, you've created a wonderful video, both pleasing to watch and informative, The swiss cheese causation model I have used as a safety professional to explain when someone says following an incident that it just seemed to be "one of those days where nothing went right." Compare this to the Eastern Air Lines Flight 401 crash in the everglades near Miami Fl. That is a situation where, basically, nothing was wrong other than an indicator light. They became so focused on the fixing the indicator, that they all missed that the aircraft was slowly descending to it's demise. Much can be learned from these incidents, and you did a great job explaining this. Well done! 1 Always Aviate, then Navigate, then Communicate. And never be low on Fuel, Altitude, Airspeed, or Ideas.
