The NAV hold is to be used when you are enroute, and the APR hold when you use the ILS and you want to automatically descend. The NAV could be used during the approach, but it will only keep you lined to the runway, without managing the altitude.

The NAV switch in some models switches between GPS and NAV(COM) for A/P reference. In advanced aircraft with a full FMC/FMS package then there is usually LNAV, VNAV which lock to headings and altitudes in an FMC, then VOR-LOC to lock to a VOR or localizer, and APP to lock to a localizer and glide slope for an ILS system which has both.

Short answer - NAV mode is used to follow VOR's, and APR mode is used to follow ILS's.

    Hope this helps,

Not exactly, because NAV can also be used with the GPS.

NAV mode will control the horizontal steering of the aircraft based on input from the GPS or NAV radio.

APR mode will control both horizontal and vertical steering of the aircraft based on input from the NAV radio.
It will delay vertical control until it intercepts the glideslope from below.
