Use the default keyboard commands?
Keyboard is one option.
Are you still able to use the rudder and right toebrake?
IIRC you can map the right brake as just 'brake' and then the sim will translate 'rudder + brake' into 'left & right brake'.
You could construct something that you can press with your upper left leg.
If you mount two (sliding) pots side by side you can manipulate them with two fingers.
A spring or a bit of elastic would pull them back forward.
You can also mount a set of paddles on the yoke.

I don't know if this is an option, but the CH Eclipse has rudder paddles on the yoke...
You could set the trigger buttons on each handle for the brakes, then operate the rudder paddles with your thumbs.

Joysticks like the Saitek Evo have twist grips, which allow you to map the yaw axis to that... simples. perfect for someone with restricted reach.
