Then YOU are doing something wrong.
I have NEVER had any issues with obtaining an "authorisation code" from RC - several times over the course of system builds, alterations and most recently with the purchase of a new laptop.
Their service has never been worse than second to none

No it doesn't. FSX ATC is quite realistic and sophisticated - IF you take the trouble to LEARN how to use it. Which frankly is easier than using real world ATC in my flying experience.
If you insist on using the novice hand holding mode - FS ATC is going to be pretty dumb - because that is what you request. If you want to use real world approaches, transitions and such - then you have to learn how to ask for alternatives, and enough navigation to fly the approaches without hand holding.
Look at the process described above.
No there is not.
FS uses a worldwide general system of approach assignment. The Approach Assignment is the key to the FSX ATC. But as mentioned above - winds and runways create a scoring system. In "light and variable winds" which is under 8 knots - the lower numbered runways are going to be the runways in use.
For airports like KLAX and KLAS, if you want real world runway assignments - set the weather for higher winds from the proper direction.
That is the control mechanism FS gives you to make your flying 'realistic'.
Flight Simulator is not an Airport Simulator - it gives you a lot of control, but it has never been designed to match all the thousands of different usage rules for every individual airport in the world.
For that VatSim is the only way to go.

I use Radar Contact and it has ATC select the correct rwys for take-offs and landings. I also has the AI traffic departing and landing on the correct rwys with relation to the wind.
You can also request VFR clr, even from uncontrolled airports, it will telephone your request, then give you a time-stamped departure and once airborne you contact ATC and announce your intentions.
I also enjoy Vatsim, the ONLY problem with Vatsim is if there is no ATC online you're back to using Unicom and if there are no planes in your area you're flying by yourself. I check Vatsim (vatspy, servinfo, etc) prior to flying, if there is no ATC coverage or little to no traffic I stick with Radar Contact, it's always on.
Just my 2 cents.
I have the same problem. I was landing at MSLP and when i had no fuel, the closest runway was Runway 25 but the ATC directed me to circle to land on Runway 7 and i ended up crashing. I am trying VATSIM but i have difficulty on downloading it and knowing what to do. I might post something asking how to do it...
