  2. Tim, Your way of doing is OK in a flightsim, but in real life the ATC will assign the altitudes depending upon the airspace and traffic. Pramod
  3. John, I am a real-life instrumented pilot and to answer your question - the approach is typically activated only when the air-traffic-controller clears the pilot for the approach. Typically, this happens when the plane is 5-8 miles from the first waypoint on the approach (called the Initial Approach Fix or IAF). For jetliners this distance could be greater due to higher speed.
